Child and Teen Checkups (C&TC) Program

Comprehensive health care for children and teens, birth through age 20, who are enrolled in medical assistance (e.g. South Country Health Alliance, UCare, Blue Plus, Medica) or MinnesotaCare.

Family Connections Program

A home-based parenting program for expectant women and families with children 0-3 months. Program works with multi-stressed overburdened families of children until they graduate the program with their children, usually when the child reaches 3-5 years of age.

 

Follow Along Program
Computerized developmental screening is offered to children birth to 36 months - no cost to program participants.

Maternal Child Health Program (MCH)

Offers counseling, nursing services, education for pregnant moms, new moms, babies and chronically ill children.
 

WIC Program

Provides nutrition education and supplemental foods for pregnant or breastfeeding women and for infants and children up to 5 years of age. Eligibility is based on income and medical need.
 WIC

Counseling and educational opportunities are available for young parents

Family aid is offered for parents with special needs.

Interpreting service is available when needed
